In September 1994, what innovation helped to eradicate a disease in the Americas, yet sadly still persists today in some countries?

The inactivated polio vaccine, developed by Jonas Salk. In addition to his and others’ vaccination developments in the 1950s, the poliovirus was eradicated in the Western Hemisphere.
